Please download, print, read, and fill out the New Patient Packet (all 9 pages), Missed Appointment Policy, and then bring them with you to our first appointment.
Please do not send these forms to me by e-mail. E-mail cannot be guaranteed to be secure, in order to ensure the privacy of your personal information, I ask that you complete these forms and bring them with you to your appointment.

I offer a free phone consultation to help relieve any anxieties, answer any questions, and allow you to find out if I would be the right fit for your needs and preferences. Please contact me at number provided above.
Services are provided Monday through Friday, appointments are available for mornings, afternoons, and evenings.
NOTE: A fee of $85.00 will be charged for missed appointments unless you call 24 hours in advance or there are extenuating circumstances.

Directions & Location:
My office is located at 146 Wind Chime Court Raleigh NC 27615 just off Six Forks Rd. in Jamestown Square II, 3/4 mile north of Lynn Rd. or 2 miles south of I-540.
The office is on the first floor of a beautifully renovated two story office condo. It has a spacious waiting room and offices with lots of natural light.
There is plenty of free parking in a well-lit parking lot and other healthcare professionals occupy the surrounding offices.
Both the building and my office are Handicap Accessible.

I can provide out-of-network services with other insurance companies. If your plan is not listed, call my office and I will help you figure it out. As a courtesy to you I will file insurance. Before your session, it will be your responsibility to contact your insurance carrier to request the following information:

Number of visits per fiscal or calendar year
Authorization (if needed)
Payment or co-payment is expected for each session with the exception of EAP (Employee Assistance Program) visits. I may be willing to negotiate a fee adjustment or payment plan in circumstances of severe financial hardship.
